DS-82 US Passport Renewal Application

Complete the Passport Renewal Application form online, via the Department of State website and print and sign it with black ink.

  • Enter all information requested- missing information can cause delays
  • The printed application must contain a 2D barcode in the upper left corner Handwritten applications ARE NOT accepted by the US Passport Agency 
  • All applications require an original signature, e-signatures are not permitted
  • Note that the new passport could be shipped back to the address on the application directly from the US Passport Agency. Please make sure this is an address that can receive UPS shipments (no P.O. Boxes).
  • The completed DS-82 Passport Application form will open as a PDF document. Do not alter this PDF if changes need to be made. Please recomplete application online and print again.
  • Print this form single sided, and sign in black ink. Do not date the signature line until the day you are ready to send Swift your application. Swift has a short window to submit your application from the day it is signed.
